How do you use forms on the website?

The form on the site asks for an email, an unticked box and the reason: menu, events or jobs. Do not add an extra field if the page only needs the address of someone already looking at the venue. Say which page holds the form and which list it feeds. The miss is a ten-field form nobody finishes on the phone, between the metro and the door.
